Examination of Vaginal Wet Preps

music It's a typical day. A patient has noticedsome itching, or maybe an unpleasant vaginal odor. During her exam, the ian willcheck vaginal pH, examine any discharge that's present, and collect a sample. Then, it'son to the microscope. music This is where you'll gather more specificinformationabout what's causing those vaginal symptoms. We'll show you how to prepare andexamine vaginal wet preps and how to do a whiff test. The results, combined with the patient's vaginal pH test, will aidthe diagnosis.

Under the microscope, you'll be looking for trichomonads, yeast, and the clue cells associated with bacterial vaginosis. We'll show you how to recognize them. music First, the microscope itself: This is a compoundlight microscope. It has several objective lenses on a rotating mount. For our purpose,one of these has to be a 10x low power objective, and one has to be a 40x for greater magnifiion.This flat part, under the objectives, is the stage. Under the stage is the condenser. Belowthat, at the base of the microscope, is the light source. There are two knobs that controlfocus; one for coarse adjustment and one for

fine adjustment. And these are the oculars,or eyepieces. We'll come back to the microscopea minute, but first, let's look at how to prepare wet mount slides. The complete vaginal wet mount involves botha saline prep and a potassium hyoxide, or KOH, prep. When the vaginal sample was collected,the swab was placeda test tube with approximately half a milliliter of saline. So, for the salineprep, you only have to take a op of the suspension and place it on a slide. Add acoverslip, being careful to avoid trapping air bubbles. Your saline slide is ready.Place a second op of the vaginal sample on another slide and add one op of 10 percentKOH. Sniff the preparation immediately, using

your hand to waft any odor toward your nose.This is the whiff test. Note if there's a fishy or amine odor. Then add a coverslip,avoiding air bubbles. Keepmind that you must work quickly to prepare and examine thewet mounts. That's because trichomonads may lose their characteristic motility within15 to 20 minutes. Before we move on now, though, let's lookat the cast of characters you may discover. These are normal squamous epithelial cellsfoundthe vagina. They're large, flat cells with a small nucleus and a large area of cytoplasm.Note that there is some granularitythe cytoplasm.

Polymorphonuclear leukocytes are known asPolys, or PMNs. They may also be called white blood cells, or WBCs. These are small roundcells. Several lobes of the nucleus are visible within the surrounding cell cytoplasm. Findingmany PMNs may indie infection. Trichomonads are pearshaped protozoa whichmove by means of flagella. Trichomonads are similarsize to PMNs and are identifiedby their characteristic jerking movement. The actual flagella may be too thin and toorapidlymoving to be seen. A clue cell is a squamous epithelial cellcoated with enough small bacteria that at least 75 percent of the cell's border is obliterated.It may look as if someone has spread glue

over the cell and pressed itsand. Cluecells are associated with bacterial vaginosis, a conditionwhich the normal microbialflora of the vagina is disrupted. Yeast may be foundtwo forms. Pseudohyphaeare the long, tubular, branching forms. Budding yeast are paired yeast cells that resemblea shoe print. The larger part is the sole and the smaller bud is the heel of the shoe. The saline prep will allow you to see epithelialcells, PMNs, trichomonads, and clue cells. You can also see yeastsaline, but sometimesit's hidden by epithelial cells or by PMNs. Red blood cells, sperm, and bacteria can alsobe seen.

I'm going to talk about grains and starchy foods today. I don't know howmany tutorials I've seen. How many blogs I've looked at and how many people I've talkedto who think all grains are completely unacceptable on the yeast infection diet. They think youcan't eat bread. You can't eat potatoes. You can't eat starchy foods. That somehow thesefoods are toxic. They make you sick. They may turn you into some sort of weirdo or youturn into a Frankenstein overnight. Some sort of crazy stuff like that.But I can tell you now, this is a load of

crap. I've learned from experience that youneed to try these things for yourself because one size doesn't fit all. By heating and modifyingthe starch, it can change the way that the gut reacts to the starch. It can change howit ferments the starchthe bowel, how it breaks it down, how it absorbs it. I learnedthis a long time ago with bananas. I thinkNew York they say bananas.We know that bananas are not a great thing to eat with Candida. But I can tell you now,green bananas are usually perfectly fine. If you get a plantain banana or the greenones, you might have seen them sometimes at the markets. Go to the markets where the Samoans,Hawaiians or the Tongan people go to, the

big people, the wonderful people with greatsmiles and play good music. Those people often eat Taro and yams and green bananas and fish.The green bananas tend to be okay with Candida, especially if they're cookedcoconut milkand I've not found that to be a problem with a lot of people. So that blows your theoryright out of the water. You can't have bananas with a yeast infection.But if you try to have a yellow banana, the ones grownMexico or Guatemala, the tropics,that go really yellow and brown, they can make you feel like crap. You can get bloating,gas and headaches out of those. They can also create strong histamine like reactions inthe body. Some people can develop strong allergies

toward bananas, depending on the stage ofripeness when they're eating them. And that depends a lot on the sugar content and chemicalcontent. Because there are lots of different chemicalsfruits and vegetables.Cornell University did research on this years ago, looking at fruits and vegetables andthe chemical contents and how they affect humans. Any kind of vegetable or fruit willaffect you different depending on the stage of ripeness, i.e., how it's picked when youeat it. And also how it's processedterms of heat. When you read something on a specificcarbohyate diet, SCD, for example, book or the Gaps books when they say, quot;You can'teat this. You can't eat that.quot; You need to

sort of look at it and think, quot;Well, whatabout if I put chick peasthe pressure cookeré What about if I bake potatoes at ahigh temperatureé How will they sit with my tummyéquot; Lots and lots of people have comeback to me and said quot;Eric, that's funny. When I pressure cook chick peas, I can eat themperfectly well. But when I boil them and add hummus, I had gas like an elephant. I wasfarting the whole day like bull frog.quot; That could be your problem.If you've got a lot of gas because of a starchy food, think how it was cooked. Was it warmwhen you ate ité Was it cold when you ate ité How was it modified by heaté How was itcookedé If it was a fruit, at what stage of

ripeness was it pickedé How was it preparedéThat's a very interesting food for thought for you, so think about those things. Beforeyou discard a food immediately because it's starchy or a grain, try to modify it. Seewhat happens to the tummy. See what the bowel motion's like. See what the gas is like. Today I'm going to share with you my top tips for gettingrid of bad breath fast, including the diet and supplements you can do, along with usingcertain essential oils. Bad breath is something that absolutely nobodywants and can sometimes be a sensitive topic. But if you want to take care of bad breath,first we need to get to what is the actual cause of bad breath. Typically, it's causedfrom the bad bacteria buildup within your body. Now, sometimes certain foods like garliccan give you temporary bad breath, but chronic bad breath on a regular basis is caused fromyeast and candida overgrowthyour body.

So the first thing we've got to adess isgetting rid of yeast and candida. You're going to do that by following a dietthat is lowersugar and higherprobiotic rich foods. And so, again, if you're consumingtoo many, not just processed sugar, but grains and white bread and white rice and even wheatproducts, but grains that over time, starches that break down into sugar, that's the biggestculprit and problem. Really yeast and candida feed off of sugar and grains. So I would recommend for a temporary timegoing completely grain free and eliminating sugar from your diet and replacing those foodswith lots of protein, fiber, healthy fats,

and probiotic rich foods, and, last but notleast, alkalizing foods. Some of the best probiotic rich foods you can addare thingslike 24 to 29 hour fermented homemade yogurt, buying goat's milk kefir, sauerkraut, kimchi,kombucha, coconut kefir, but really loading up on the probiotic rich foods. That goodbacteria is going to help overcome the yeast and candidayour body and help balanceout those microbesyour body. That's going to help breath very, very quickly. Also, again, consuming more healthy fat. Healthyfat, especially coconut oil. Coconut oil contains specific types of fatty acids called mediumchain fatty acids, like lauric acid, capric

acid, and caprylic acid, which are antimicrobialin nature. So doing a tablespoon or two of coconut oil a day or eating a lot of coconutproducts can help bad breath as well. And then, the next thing you want to do isthere are certain foods and supplements you can take to help out with bad breath. Again,the first is a probiotic supplement. The second thing you can do is use peppermint essentialoil. In fact, I make my own athome toothpaste with a mixture of baking soda, coconut oil,and peppermint oil. Just a single op of peppermint oil on your tongue oryour watercan help freshen up your breath. Then a food that can really, really help inimproving your breath is parsley. Part of

the reason is parsley is so alkalinenature.So if you are making vegetable juice at home, consuming vegetable juice can actually helpgive you fresh breath, especially if you're using things like parsley and green leafyvegetables like kale, spinach. and chard, also doing things like cucumber and celeryjuice. But if you start doing a green juice daily and also consuming parsley, those arespecific foods that will have a profound effect on improving your overall breath.
